Information for Clinic Appointments

We are located in North Austin at 13915 N Mopac Expwy in a four story building named ‘Enterprize Plaza' with 'WACHOVIA’ sign on it.   There is ample free parking available. 

If this is your first visit, please print and complete the ‘Clinic Forms’ listed below.  If you are unable to do so, please arrive 30 minutes prior to your scheduled appointment time to fill out these forms.

Patient Forms

If this is your first visit with us, please print and fill out the following forms.  This will expedite your visit.  Please bring the completed forms with you to your scheduled appointment.  Please bring a copy of your medical records and test reports with you if possible.  If you have visited other facilities and would like your records from those facilities to be faxed to our clinic, please fill out the Authorization for Release of Medical Records form included in the patient forms and bring it with you.

What to Bring to Your Appointment

      Your insurance card or other insurance information

      Physician referral forms if required by your insurance carrier

      Above Patient Forms including Patient Registration, Patient History Questionnaire, Consent for Care, Financial Policy, Consent For The Use & Disclosure Of Health Info, and Authorization for Release of Medical Records if applicable

      An up-to-date list of medications with the dose and frequency if not described on the Patient History Questionnaire form

      Any test  reports including EEG, EMG, NCV, CAT Scans, MRI, and Angiograms

In case of an emergency:  If you have an emergency prior to your initial appointment, please contact your primary care physician, or call 911 to get immediate medical attention. 

Rescheduling or Canceling an Appointment:   Please call us as soon as you decide to reschedule or cancel your appointment.  Please let us know at least 24 hours in advance.

If you are late: Please call us and let us know.  We will try to accommodate but you may be rescheduled.
